The following Forms are in Microsoft Word and compatible with most versions, specifically Word versions 2000-2006. When you click on the above links, you will be prompted to accept the upload of the file to your computer. (You may have to modify the settings of your web browser to allow for this transfer.)
The forms are fill-it-in. Simply tab or enter to go to the next field. When complete, you may either 1) Print and turn into the office, 2) Save to your computer and then attach to email as an attachment and email the file back to the office at lifecm@aol.com, or 3) fax printed completed form to the office at 909-882-2898. Please call the office within the next 48 hours to ensure your request was received.
